Professional Experience Forms

Placement Forms

We have recently introduced a new system whereby placement forms are now completed online via SONIA and have discontinued the use of paper based placement forms. You will be contacted two months prior to your scheduled placement and asked to visit SONIA and complete these forms at least one month prior to the start date of your placement. Placement forms submitted after the deadline may not be allocated a place.

Please follow the instructions below:

  • Ensure you enrol into your professional experience unit via MyInfo
  • Go to ‘SoniaOnline’ SoniaOnline and select ‘Electronic Forms’
  • Complete your electronic placement form, please ensure that you click ‘Submit’.

If you have any queries please email professionalexperience@murdoch.edu.au (please include your name, student ID number and if you are a Primary/Secondary student in the subject line).

Name Badge Order Form here. Payment of $10 or $16 with postage, to be made in person to the Student Centre at Murdoch South Street campus (not to the Professional Experience Office) OR by post to the Student Centre.

Notification of Absence from Professional Experience

If you are absent for any part of your school Experience, this  form needs to be provided to the Principal or School Experience Coordinator to complete. At the conclusion of the Professional Experience prac, the form needs to be submitted to the Professional Experience Office, together with the student's final Evaluation Report.

Checks and Clearances

There are several checks and clearances that students must provide before undertaking any school experience at a Western Australian school. It is the responsibility of each student to ensure that their clearances and checks are up to date.
