To make an appointment at the Skin, Allergy and Ear Clinic, you will need to make sure your vet has completed a referral form, which you can bring with you or your vet can email or fax to us.

A questionnaire regarding your pet for you to fill out can also be very helpful for us. You can download the questionnaire (pdf format), which you can either fax to us or bring with you at the time of your appointment.
