2017 Nomination Form

The Nomination Form for the 2017 Distinguished Alumni Awards is now online. The form is being released early to allow sufficient preparation time for nominators. 

You can nominate yourself, or another alumnus who has made an outstanding contribution in their field. 

To submit an eligible nomination you must do the following:
1. Download and prepare a nomination form (Word & PDF forms here)
2. Prepare or obtain a current CV of the nominee
3. Prepare any additional materials (e.g. press coverage) that support your nomination
4. Submit the prepared documents using the Online Submission Form during the Submission period 5 April 2017 until (AWST) 5 pm 31 May 2017
