The Get Engaged evening will discuss new developments in small animal surgery. Professor Giselle Hosgood, together with Dr Carla Appelgrein and Dr Isobel Monotti, will deliver an evening of insights into the management of distal limb cutaneous defects using Seed Grafts, provide information on JPS and decision making for hip dysplasia and present a new approach for USMI.

Seed Grafts for distal limb defects

Seeds grafts are a form of free skin grafting that offers an alternate to second intention healing, when other reconstruction such as skin flaps, pouch flaps or mesh grafts are less applicable for reasons such as wound size (small), location, movement, low grade infection, and graft bed irregularity. It is particularly applicable to distal wound defects and is a robust, straightforward, low cost procedure with a functional outcome. The grafts are applied to wounds after trauma or mass resection. The technique will be described with case examples illustrating indications and outcomes.

A new approach to urethral sphincter mechanism incontinence

Urethral sphincter mechanism incontinence (USMI) is caused by a decreased urethral closure pressure. It has been reported to affect up to 20% of female spayed dogs. Historically it has been thought that early ovariohysterectomy plays a role in the development of the condition although this has not been proven and the actual cause is likely complex and multifactorial. If urethral sphincter mechanism incontinence is suspected, medical management with phenylpropanolamine or stilboestrol is usually instituted with resolution of clinical signs reported in 85% of treated dogs with a decline in response as the dog ages. Medical management requires life-long administration of medication which can be challenging and failure to administer medication results in return of clinical signs. Additionally, some dogs do not respond to medical management.
Previous surgical techniques described to manage USMI have been met with poor resolution of incontinence and failure to maintain long term clinical results. These include techniques such as colposuspension, urethropexy and urethral bulking agents with an average reported initial success rate of 50%. All techniques have a decline in efficacy when assessed one year later. In 2004, hydraulic urethral occluders were introduced. This new management strategy has a 90% success rate with minimal minor complications encountered. It also eliminates the need for ongoing medical management.
This presentation will introduce the surgical management of USMI with a urethral hydraulic occlude and discuss placement, outcomes and complications. It may offer a favourable alternative to medical management given its good outcomes without the need for life-long ongoing medication.

JPS – juvenile pubic symphysiodesis and Early Decision Making for Dogs with Hip Dysplasia

Hip dysplasia is a common orthopaedic condition that affects dogs of all sizes. Hip laxity is the hallmark sign of hip dysplasia, leading to secondary osteoarthritis and often resulting in debilitating pain and a poor quality of life. Many surgical options for hip dysplasia have been described and the suitability of some of these procedures are dependent on patient age and disease severity. This presentation will focus on the early detection of hip dysplasia, the indications for JPS and an outline for decision making for dogs with this disease.
